Welcome to the St. Francis Diabetic Self-Management Program. This free program is designed to provide you with the tools and skills needed to take control and manage your diabetes. This program is designed to create better care for individuals including access to care, quality of care, health outcomes, and improved health.

Group visits are held in the cafeteria of St. Francis Medical Center
Diabetes Support Groups are held at Trinity Cathedral Church from 12PM-2PM every other Friday. The next Support Group is December 4, 2015. Call to register at 609 599 6215.
